At Family Medical Practice it is free to take advantage of a “Cashless“ Direct Billing service, and we do not charge you a membership fee to gain access to this. As people’s circumstances are always changing, we would like our customers to complete our “Payment Scheme” form upon registration. In order for us to establish direct billing, it is now necessary to present your valid credit card at Family Medical Practice. Family Medical Practice staff will then notify customers of any excluded and declined amounts, and are asked to be responsible for the payment within 14 days. If this amount is not settled within this time the payment scheme will be used to avoid disruption. All payment scheme information is treated as highly confidential. Please be aware that Family Medical Practice do not provide direct billing service for any obstetric/pregnancy related consultations, tests and prescriptions. We ask for your understanding on this matter and please kindly self pay and claim the costs from your insurance company directly. Our doctors are always happy to complete claim forms and write medical reports if this is a requirement of your insurance provider. This service is free of charge, but please allow several days for this service. Our reception staffs are available 24 hours a day and can help you to download and print claim forms, contact insurance companies for clarification of your policy and are happy to give advice on any issues, big or small.
