Clinic-Policy.jpgAt Family Medical Practice we offer  “Cashless“ direct billing services for patients with a variety of insurance plans, and we do not charge a membership fee to gain access to this.

As people’s circumstances are always changing, we would like our customers to complete our “Payment Scheme” form upon registration. In order for us to establish direct billing, it is now necessary to present a valid credit card at Family Medical Practice. Family Medical Practice staff will then notify customers of any excluded and declined amounts, and request payment within 14 days. If outstanding payment is not settled within this time period, the payment scheme will be used to avoid disruption. All payment scheme information is treated as highly confidential.

We ask for your understanding in this matter and you may always self-pay and claim the costs from your insurance company directly if you do not agree to the terms of the agreement.
