Required Documents

Please bring any of the following documents with you for verification and registration:

FOR PRIVATE PATIENTS

Insurance Card—Letter of Guarantee from your insurance company to cover the wellness benefits, if applicable.

For the eye tests, those who use glasses should bring them for the scheduled vision.

Please bring along your most recent health check-up report, mammogram film/report, and treadmill report (if available) should you wish to have a comparison made between that and the latest result.

FOR CORPORATE CLIENTS

Please bring along your ID or insurance card (if required from your HR) and your valid passport and three photos with a white background (size 4 cm x 6 cm) if you want to combine your Work Permit at the same time.

FOR WORK PERMIT

Please bring your valid passport and three photos with a white background (size 4 x 6 cm).

If you have a chronic illness or other health problems, please bring any test results or medical report with you to assist in diagnosis.

FOR VISA APPLICATION

Please bring your Immigration department medical form (if any) and your valid passport or identity card/citizen identification for identification.

Prescription eyeglasses or contact lenses that you might wear is required.

You need to bring documents that related to your previous medical conditions.

Please come along with your parents if you are under 16 years old.
